Work Shifts 8:00 A.M - 4:00 P.M Job Description SUMMARY OF DUTIESAND RESPONSIBILITIES - The Public Health Educator will be responsible for entering data and maintaining the Pediatric Asthma Registry for the NYC H+H/ Kings County team as a continuation of the NYC H+H Pediatric Chronic Diseases Collaborative. The Public Health Educator will also be trained to produce monthly "run sheets" of all registry data and will participate in preparation of the monthly leadership report.
- The Public Health Educator will assist the Pediatric Asthma Nurse Practitioner with her Inpatient Screening and Outreach project (data collection, bedside education, letters to community physicians, arranging for appointments, etc.).
- The Public health Educator will assist with patient/ parent education during all Pediatric Asthma and Allergy clinic sessions. (Asthma sessions are held Thursday mornings, and Friday afternoons. Allergy sessions are held Tuesday and Friday mornings). The Public Health Educator will also assist with education in the Pediatric Chest/Pulmonary Practice held on the second and fourth Wednesday of every month. If additional sessions are added to the program, the Public Health Educator may be assigned to assist in education for these panels as well.
- The Public Health Educator will attend all Pediatric Asthma team meetings and will be an integral part in planning all team activities and programs.
- The Public Health Educator will check messages left at the Pediatric Asthma Hotline number (245-5518) and process/ forward them as indicated to the appropriate parties.
- The Public Health Educator will be responsible for selecting, maintaining and restocking all written educational materials as well as self-management supplies (spacers, peak flow meters, etc.) in the consultation room. The Public Health Educator will also be instrumental in helping the team to determine which educational materials are effective and if any additional resources would be helpful to the patients.
- The Public Health Educator may be involved in additional programs and projects depending on any future grants, and health care initiatives.
- Provide reinforcement of asthma education that is interactive and tailored to patient's and family's needs.
- Communicate with primary care team, specialty providers, and care managers (including Health Home and managed care plans) as needed.
- Refer to care management or social workers for complex needs, schedule patients and make reminder phone calls. Document into the EMR.
Duties & Responsibilities 1. A masters degree from an accredited college or university in public health education, education, public/community health administration, public administration or business administration, and one year of full-time satisfactory experience in developing public health education programs, including identifying target populations, conducting needs assessments, designing educational materials, planning educational presentations or workshops,
evaluating health education programs; presenting public health education programs; and/or counseling in areas such as communicable diseases, substance abuse, assault, sexual abuse, and/or family planning; or,
2. A baccalaureate degree from an accredited college and two years of full-time satisfactory experience as outlined in one (1) above.
